Patricia A. Hernandez, 66, of 5313 Greenbrook Drive, Portsmouth, transitioned Thursday, July 30, 2015, at Kindred Transitional Care and Rehab-Nansemond Pointe.

She is the daughter of the late Louise Colbert Jolly and Samuel B. Cheeks. She was a graduate of John F. Kennedy High School class of 1967.

She leaves to cherish her memories her husband, Ramon Hernandez; five sons, Demetrius Holland, of Jacksonville, Fla., Troy Holland, of Suffolk, Michael Holland, Ranaldo Hernandez and Ramon Hernandez Jr. all of Portsmouth; brother, Samuel Cheeks Jr. (Connie), of Suffolk; devoted cousin, Gloria J. Colbert, of Suffolk; five grandchildren; numerous cousins, family and friends.

A graveside service will be held at noon, Aug. 4, 2015, in Carver Memorial Cemetery. In lieu of flowers, please make a donation to the American Cancer Society.
